The BP Deepwater Horizon Macondo well blowout, explosion, fire, and oil spill was the most significant economic, environmental, and societal disaster of 2010. What caused it? At the request of the US Secretary of the Interior, Kenneth Salazar, the National Academy of Engineering investigated and released a report on the causes of the Deepwater Horizon oil rig blowout and suggested ways to prevent such a disaster from happening again.
